Job Title: Registered Nurse Job Specialty: Case Manager Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Not Specified Experience: Must have acute care experience License: Registered Nurse License Certifications: Not Specified Must-Have: Acute care experience required Job Description: - Coverage needed for an adult unit during maternity leave. - Schedule will primarily be Sunday through Thursday but may change based on staffing needs. - Traveling Case Manager's schedule subject to change. - Participate in admission criteria assessments, including options for drug/alcohol abuse treatment. - Work with patients experiencing homelessness and those with mental health histories or crises. - Familiarity with advance directives, POLST, adoption, and foster care processes. - Ability to organize and participate in family meetings. - Experience with HMOs, IPAs, and similar organizations. - Competent in arranging DME, SNF, and ambulance transport independently. - Participate in the development of treatment plans. - Understand the protective services process. - Required to float among all facilities within a given state, refusal may lead to assignment termination. - Communication devices will be provided for work duties if necessary. - Pre-start modules are non-billable, averaging 16 hours; completion is required. Orientation: 40 hours; non-billable
