A company is looking for a Transitions Coach Registered
Nurse.
Key Responsibilities
Empower patients to engage with the healthcare system and achieve
personal goals
Support patients in understanding their post-discharge care
plans
Guide patients through their healthcare journey to ensure they feel
informed and confident
Required Qualifications
Graduate from an accredited school of nursing with a bachelor's
degree or higher
Current Minnesota Registered Nurse license
Minimum of 5 years of clinical nursing experience with diverse
patient populations
Current American Heart Association (AHA) BLS Healthcare Provider
card
