Job Description:

A company is looking for a BH Care Coach to provide prevention education and coordinate post-discharge care for members transitioning from various care settings.
Key Responsibilities

Ensure comprehensive BH Care Management to improve health outcomes and support long-term recovery for members with behavioral health diagnoses
Provide care coordination for members with behavioral health disorders, monitoring treatment progress and promoting recovery skills
Collaborate with internal departments and community organizations to link members to appropriate services and ensure compliance with regulatory requirements

Required Qualifications

Bachelor's degree in a health or human services field, QMHP, MSW, or BSW
Minimum of 2 years of experience working with individuals with behavioral health diagnoses
Ability to implement structured care management for transitions of care
Must reside in Virginia or within 40 miles of bordering states
Intermediate to advanced computer skills and experience with electronic medical records
