Job Description:
A company is looking for a Case Manager Nurse - LPN/LVN.
Key Responsibilities
Coordinate patient care over the phone between providers and
caregivers
Perform assessments to determine patients' healthcare needs and
barriers to care
Educate patients on treatment options and coordinate referrals
Required Qualifications
Graduation from an accredited LPN/LVN program and a current compact
LPN/LVN license
Experience in medical, surgical, and/or acute care settings
Strong understanding of case management and health program
coordination
Ability to meet productivity and quality standards
Clear communication style and strong collaboration skills
